Adult Community Support Services

Community Support Services (CSS) are designed to help adults with serious mental health needs build stability, independence, and confidence in daily life. These services focus on supporting individuals as they work toward their personal goals and participate fully in their community.

A key part of Community Support Services is case management. Case managers work alongside individuals to identify needs, connect them with helpful resources, and provide ongoing support. Services are person-centered and tailored to each individual’s strengths, goals, and circumstances.

Community Support Services are flexible and accessible, with support available in a variety of settings, including the home, community, or workplace—wherever support is most helpful.

Who is This For?

Community Support Services are for adults who may need extra support managing their mental health while working toward greater stability and independence. These services may be helpful for individuals who:

  • Are living with ongoing or serious mental health challenges

  • Want support staying connected to treatment, resources, or services

  • Need help managing daily responsibilities or life transitions

  • Would benefit from support in their home, community, or workplace

  • Are looking for individualized, ongoing support to reach personal goals

If you’re unsure whether Community Support Services are the right fit, our team can help talk through your options and determine the best next steps.

Support through Community Support Services is flexible and personalized, based on each person’s needs and goals. Services may include:

  • Working one-on-one with a case manager to identify goals and priorities

  • Connecting with community resources such as housing, employment, healthcare, or benefits

  • Support with organization, routines, and daily responsibilities

  • Coordination with therapists, medical providers, and other supports

  • Advocacy and guidance during life transitions or challenging situations

Support may take place in the home, community, or workplace—wherever it is most helpful and meaningful.

Group sessions provide opportunities for connection, shared learning, and support. For Community Support Services clients, groups help reduce isolation, build practical skills, and encourage growth through peer support in a safe, supportive environment.

Learn simple, healthy cooking skills while building confidence, independence, and everyday life skills in a supportive group setting.

Engage in arts and crafts activities that promote relaxation, creativity, and healthy use of free time in a supportive group setting.

Community Support Services are available to adults who meet eligibility criteria based on mental health needs and clinical assessment. Eligibility is determined during the intake process and may include a diagnosis of a serious or ongoing mental health condition.

If you’re unsure whether you qualify, our team can help. Walk-ins are welcome during Open Access, where we can talk through your needs, explain available services, and determine next steps together.
