CSS Adult Case Manager

As an Adult Case Manager, you will support individuals experiencing mental health challenges in building the skills needed to live independently and successfully within their community. This role focuses on connecting individuals to resources, reinforcing treatment goals, and helping clients navigate everyday responsibilities. Through collaboration and advocacy, Adult Case Managers help individuals improve stability, independence, and quality of life.

  • Support individuals in developing daily living skills such as budgeting, nutrition, personal care, and appointment management

  • Collaborate with clinicians to support individualized treatment plans

  • Monitor progress toward treatment goals

  • Develop and facilitate psychosocial rehabilitation groups as appropriate

  • Coordinate transportation for community-based client visits as outlined in treatment plans

  • Assess and monitor eligibility for services

  • Complete documentation including progress notes and service records accurately and timely

  • Provide case management, targeted case management, psychosocial rehabilitation, attendant care, and crisis services as needed

  • Participate in on-call coverage as scheduled

  • Attend required meetings, trainings, and professional development activities

  • Perform additional duties as assigned

Requisitos

Minimum Qualifications
  • Bachelor’s degree in psychology, social work, or related field

  • Valid Kansas driver’s license with favorable driving record

  • Background free of issues related to abuse or neglect

Preferred Qualifications
  • Knowledge of community resources and support systems

  • Understanding of recovery-oriented care

  • Experience working with individuals with mental health challenges or disabilities

  • Experience with program development or service coordination
