As a Crisis Case Manager at The Center, you will provide compassionate, responsive support to individuals experiencing behavioral health crises while helping connect them to ongoing services and long-term recovery. As part of our Mobile Crisis Team and Assertive Community Treatment (ACT) program, you will respond to crises in homes and communities, provide behavioral interventions, coordinate follow-up care, and collaborate with community partners to ensure individuals receive timely, appropriate support. Key Responsibilities
Respond to behavioral health crises as part of a two-person Mobile Crisis Team serving four counties
Provide crisis intervention, behavioral support, and case management services in home, community, and office settings
Complete assessments, treatment plans, and monitor progress for individuals receiving ACT services
Conduct proactive outreach to reduce future crises and support community stability
Collaborate with law enforcement, hospitals, families, and community partners to coordinate crisis response and follow-up care
Connect individuals and families with community resources and ongoing behavioral health services
Provide transportation as needed to complete crisis visits and community outreach
Follow up with individuals after hospitalization or missed crisis appointments
Maintain accurate and timely documentation of all services provided
Participate in ACT meetings, crisis team meetings, agency trainings, and ongoing professional development
Perform additional duties as assigned
Requisitos
Minimum Qualifications
Bachelor's degree or equivalent work experience
Minimum age of 21 years
Valid Kansas driver's license with favorable driving record
Background free of issues related to abuse or neglect
Complete Case Management Training within 30 days of employment
Preferred Qualifications
Experience working in behavioral health, crisis services, or human services
Working knowledge of community and social service resources
Understanding of recovery-oriented care and crisis intervention principles
Experience advocating for individuals with disabilities
Experience with program development or service coordination
