About the Role
As a Care Coordinator at The Center, you will help individuals with behavioral health and physical health needs navigate the healthcare system and access the services necessary to achieve their wellness goals. Working closely with clients, providers, and community partners, you will coordinate care, remove barriers to treatment, and ensure each individual receives personalized, comprehensive support. This role is ideal for someone who enjoys building relationships, solving problems, and advocating for others.
Key Responsibilities:

Guide clients through the admission process, including registration, assessments, outcome measures, appointment scheduling, and referrals
Provide ongoing care coordination services based on each client's individualized treatment plan
Collaborate with healthcare providers, social service agencies, community organizations, and natural supports to coordinate care
Advocate for clients to ensure access to needed medical, behavioral health, and community services
Maintain accurate and timely documentation, including assessments, progress notes, outcomes data, and service records
Monitor client progress and assist in addressing barriers to treatment and recovery
Participate in treatment team meetings and interdisciplinary collaboration
Attend agency trainings and professional development opportunities
Perform additional duties as assigned
Requirements
Minimum Qualifications
Bachelor's degree in psychology, social work, or related field OR four (4) years of equivalent work experience
Valid Kansas driver's license with favorable driving record
Background free of issues related to abuse or neglect
Completion of Targeted Case Management Training prior to providing services
Preferred Qualifications
Working knowledge of community and social service resources
Understanding of recovery-oriented care and integrated healthcare coordination
Experience with program development or service coordination
Experience advocating for individuals with disabilities
Bilingual (Spanish preferred)
